Follow up on hospital discharge referrals before beds go to a competitor

How can a skilled nursing facility automatically follow up on hospital discharge referrals?
Every weekday morning, WebRun opens PointClickCare and finds hospital-discharge referrals that have been open for more than 24 hours with no update. It logs each one with the referring hospital, diagnosis, and payor in a Google Sheet, then posts a prioritized follow-up prompt to your admissions team in Slack so no warm referral goes cold.
- No referral goes more than 24 hours without a follow-up prompt to the admissions team
- Oldest referrals surface first so the team prioritizes the most at-risk opportunities
- Declined or admitted referrals drop off the list automatically each morning

Questions, answered
Will it contact the hospital or family directly to follow up?
No. WebRun surfaces the stale referral in Slack and prompts the admissions coordinator to act. All outreach to hospitals or prospective residents is made by your team, not by WebRun.
How does it know a referral has gone stale?
It checks the timestamp of the last status update in PointClickCare for each open referral. Any referral with no update in 24 hours is flagged for follow-up, regardless of the referral source.
What if a referral was declined or the prospect chose another facility?
It reads the live referral status in PointClickCare each morning. Referrals marked as Declined, Cancelled, or Admitted are excluded automatically.
